Wide receiver Mike Evans left Monday's game early with an Achilles injury; he had missed all of practice the prior week and was questionable to play due to a pre-existing Achilles issue and rib injury. NBC Sports Bay Area insider Jennifer Lee Chan reported Evans' Achilles tendon remained "intact" but San Francisco is preparing for him to miss next week's game.
